چکیده

The success rate of orthopaedic implants is strongly dependent on the initial fixation, but the objective intra-operative assessment of primary stability of cementless implants is still a challenge and, moreover, excessive press-fitting of the implant can cause intraoperative fractures. In a series of in vitro and in vivo experiments the change in the frequency response function (FRF) of the hip stem/femur structure during implant insertion was studied and, to better understand the influence of changing contact conditions on the resonance frequencies a finite element analysis was set up. Especially in the range of higher frequencies, the FRF change provides reliable information regarding the stability evolution of the implant-bone system during the insertion. The protocol described in this paper can be used to accurately detect the insertion end point and to reduce the risk for intra-operative fracture in total hip arthroplasty. The vibrational method based on FRF analysis can be adapted and extended to other kinds of orthopaedic implants.

In vivo evaluation of a vibration analysis technique for the per-operative monitoring of the fixation of hip prostheses

BACKGROUND The per-operative assessment of primary stem stability may help to improve the performance of total hip replacement. Vibration analysis methods have been successfully used to assess dental implant stability, to monitor fracture healing and to measure bone mechanical properties. The objective of the present study was to evaluate in vivo a vibration analysis-based endpoint criterion fo...

Determination of Total Hip Replacement stem insertion endpoint and stability assessment by vibration analysis: first experiences with per-operative measurements

The assessment of the primary stability of cementless implants still remains a subjective factor in total hip replacement (THR) and, as a consequence, the excessive press-fitting of a THR component can be the cause of intra-operative fractures. Objective information about the stability of implant-bone structures can be obtained using methods based on vibration analysis. After extensive in vitro...

Intra-operative error during Austin Moore hemiarthroplasty.

PURPOSE To examine the frequency of technical errors during implantation of the Austin Moore prosthesis in a public hospital. METHODS Radiographs and charts of 147 consecutive uncemented Austin Moore hemiarthoplasty patients were retrospectively reviewed with particular reference to intra-operative errors associated with early failure of the prosthesis. RESULTS 48% of the patients had at le...

Distal femoral fracture during primary total knee arthroplasty.

PURPOSE To review 6 cases of periprosthetic distal femoral fracture during total knee arthroplasty (TKA). METHODS Of 778 women and 691 men who underwent primary TKAs using posterior-stabilised (n=1240), cruciate-retaining (n=165), or semiconstrained (n=64) implant, 5 women and one man with a mean age of 73.3 years and a mean body mass index of 31.6 kg/m(2) sustained an intra-operative peripro...

Per-operative vibration analysis: a valuable tool for defining correct stem insertion: preliminary report.

BACKGROUND Defining the stem insertion end point during total hip replacement still relies on the surgeon's feeling. When a custom-made stem prosthesis with an optimal fit into the femoral canal is used, the risk of per-operative fractures is even greater than with standard prostheses. Vibration analysis is used in other clinical settings and has been tested as a means to detect optimal stem in...
